The inclusion of an on-line version of the questionnaire, to increase the number of ways potential respondents can complete the questionnaire;
The use of a reminder letter to prompt the maximum response rate among the sample;
The optional use of a ‘refusal survey’ to address any non-response bias in the achieved sample.
